As part of the 2025/26 GP contract, all GP practices across our Primary Care Network (PCN) are required to publish the You and Your General Practice (YYGP) Patient Charter by 1 October 2025.
The Patient Charter sets out what patients can expect from their GP practices – and what practices ask of patients in return. It aims to build a positive, respectful partnership between patients and healthcare teams.
What You Can Expect From Your GP Practice
- Safe, high-quality care.
- Dignity, respect, and compassion at all times.
- Support for your individual needs, including reasonable adjustments where required.
- A commitment to listen to your feedback and take concerns seriously.
What GP Practices Ask of Patients
- Arrive on time for your appointment.
- Be prepared with any relevant information to help your clinician.
- Cancel appointments early if no longer needed, so others can use the slot.
- Treat all practice staff with courtesy and respect.
Feedback and Concerns
Your GP practice is the first point of contact if you would like to give feedback or raise a concern. Each practice has a nominated lead (often the Practice Manager) who ensures that all concerns are followed up appropriately.
Patients and their representatives can also provide feedback through:
- Healthwatch – the independent champion for people who use health and social care services.
- Mid and South Essex Integrated Care Board (ICB) – responsible for planning and funding health services locally.
